Indications/Uses
CHD. Reduction of total LDL-cholesterol, apolipoprotein B & triglycerides in the treatment of hyperlipidemia including hypercholesterolemia, adjunct therapy for homozygous familial hypercholesterolemia & ischemic heart disease.
Dosage/Direction for Use
CHD Initially, 20 mg once daily at bedtime, may increase at 4-wk intervals to max 80 mg/day. Hyperlipidemia 10-20 mg once daily at bedtime. Homozygous familial hypercholesterolemia 40 mg once daily at bedtime or 80 mg/day in 3 divided doses (20, 20 & 40 mg in the morning, afternoon & evening). Renal insufficiency 5-10 mg.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Acute liver disease or unexplained elevations in serum transaminases. Porphyria, pregnancy, women of childbearing potential & lactation.
Special Precautions
Monitor liver enzyme levels periodically. Alcoholics & patients w/ liver disease.
Adverse Reactions
Constipation, dyspepsia, flatulence, pain, diarrhea, asthenia, nausea/vomiting & headache. Rhabdomyolysis w/ acute renal failure secondary to myoglobinuria occurred rarely & requires discontinuance.
Drug Interactions
Coumarin derivatives, digoxin, protease inhibitors, itraconazole, ketoconazole, fibrates, niacin, cyclosporine, erythromycin, verapamil, diltiazem & grapefruit juice.
